Our Cardiology Clinic supports a wide variety of patients with their cardiac needs.This includes preventative care, coronary artery disease, hypertension, heart failure, atrial fibrillation and more. We have a large group of providers that work closely with the RN's, clinical support staff, scheduling staff and more to provide excellent patient care.We are excited to add an individual to our clinic who enjoys being part of a team and has a passion for caring for patients! The Care Coordinator works to maximize the patient's health outcomes and experience by identifying needs, completing pre-visit planning, supporting clinic visits, and providing closure to clinic visits. This role also entails utilizing coaching strategies to help ensure patient and family participation in treatment plans, as well as providing exceptional service to internal and external customers.
