SUMMARY
Grady Health System is seeking a compassionate and experienced Sickle Cell Nurse Navigator to join our team. This critical role supports patients living with Sickle Cell Disease through personalized care coordination, education, and barrier management throughout the continuum of care.
The ideal candidate will have experience with Sickle Cell Disease or chronic disease case management, a strong background in patient education, and a passion for improving health outcomes for underserved communities.
KEY RESPONSIBILITIES
Clinic Presence & Support



  • Provide nursing support during assigned Sickle Cell clinic days.


  • Perform pre-visit self-care assessments with patients.


  • Provide education and support for patients receiving apheresis, blood transfusions, Hydroxyurea, Hydrea, and wound care.



Assessment & Barrier Management



  • Review and manage clinical barriers identified via PHQ-9 and SDOH (Social Determinants of Health) tools.


  • Coordinate referrals to Sickle Cell therapist for elevated PHQ-9 scores.



Patient Education & Discharge Support



  • Provide patient and family education on Sickle Cell Disease diagnosis and treatment plan


  • Assist patients with discharge planning, medical records transfer, and care continuity support.



Care Coordination



  • Serve as the primary nursing contact for patients transitioning from Children's Healthcare of Atlanta (CHOA).


  • Follow up on ambulatory referrals for designated patients.



Team Collaboration



  • Actively participate in Sickle Cell multi-disciplinary team meetings.


  • Contribute to development of clinical action plans in EPIC for selected patients.



QUALIFICATIONS
Required:



  • BSN Degree

  • Current licensure as a Registered Nurse in the State of Georgia

  • Three years of direct nursing experience and demonstrated clinical competence


  • Current AHA BLS certification


  • Demonstrate competence in clinical specialty (Sickle Cell)


  • Utilize advanced clinical thinking and analytical skills that contribute to patient adherence of treatment protocols and symptom management


  • Competent in use of Electronic Medical Records



Preferred:



  • Sickle cell or chronic disease case management experience
