The Clinic Nurse is responsible for managing and providing care to the patient and his/her family, and for directing and supervising nursing care provided by other members of the patient care team. The Clinic Nurse is responsible for assessing, planning, implementing, coordinating, delegating and evaluating the nursing care of patients in alignment with the professional practice model. 95% Duties and Responsibilities: *Health, Care Delivery:* Participates in our innovative healthcare delivery model that delivers high value care with unparalleled patient experience Applies the nursing process to all areas of practice Establishes a patient plan of care; provides patient education related to patient's condition, therapeutic regimen and self-care measures; evaluates effectiveness of patient education Provides care according to patient needs; performs specialized nursing care procedures as outlined in department/unit specific competency profiles Provides interdisciplinary communication to enhance the patient plan of care Enhances patient experience with high value care Actively participates in performance improvement initiatives positively affecting quality and safety Responds to urgent situations and assumes an active role to enhance emergency care needed for positive patient outcomes Adheres to regulatory compliance standards *Talent and Culture:* Supports an environment to attract and develop a diverse and talented team of people committed to creating a culture of innovation and discovery.
