Minimum to Midpoint Pay Rate:$31.78 - $42.90 / hour

Summary

Lee Physician Group is seeking an experienced RN to join their team at The Sanctuary/Regional Cancer Center in Fort Myers. The ideal RN will have at least 3 years of RN experience and oncology experience would be beneficial. This RN will work M-F, no holidays no weekends. This RN will predominantly be connecting with patients via phone, but there will be an aspect of in person patient care. This RN will be an integral part of the provider and patient team.

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.

Requirements Educational Requirements

Degree/Diploma Obtained	Program of Study	Required/ Preferred	and/or
Associate's	Nursing	Required	and
Bachelor's	Nursing	Preferred

Experience Requirements

Minimum Years Required	Area of Experience	Required/ Preferred	and/or
3 Years	Clinical Nursing	Required

Additional RequirementsMinimum of 3 years in clinical setting with emphasis on patient education and chronic disease management; home health, skilled nursing, medical surgical nursing required. Care coordination experience preferred. State of Florida Licensure Requirements
